Jury Forms

Performers must fill and print copies of their own jury forms. Bring all necessary forms to your jury performance. Utilize the college printing services that you pay for through tuition. 

  1. Print 1 copy of a jury form in your major instrument

  2. Fill out the form

  3. Make 10 copies of filled jury form

  4. Print and fill out 1 Repertoire List

  5. Bring everything to your jury performance
